Complaints Procedure

It is important for us to know if your appointment doesn’t quite go to plan.

A complaint is a situation when you feel your rights as a client have been breached or anything else that makes you unhappy.

It is OK to complain or to make a suggestion if you feel things didn’t go quite right at your appointment.  It gives us an opportunity to improve our service and it will not have an adverse effect on the way you are treated in any way.

You can use the form below to submit your complaint or suggestion (this is forwarded straight to management), otherwise ask for a form at reception.  Alternatively if you would like to speak to someone in person you can book an appointment at reception with our Practice Manager.

All complaints and suggestions are taken seriously and will be handled with complete privacy, dignity and respect.

You can contact the Health Advocate Trust on (09) 6389638 or the Commissioner on 0800 11 22 33 or Health and Disability Services at any stage of this complaints process.
